Appointment Sheet

What you want to say to the doctor, written in advance. Hand it over if the words do not come.

Waiting rooms, time pressure and eye contact make it hard to say what matters. Writing it beforehand is not cheating, it is good medicine: doctors get clearer information and you leave with fewer regrets. You can hand this sheet over and say 'it is all here'.

What is inside

  • Why I am here today (one or two sentences)
  • How long this has been going on, and what changed recently
  • What I want to describe
  • Symptom or difficulty · How often / how bad (0-10) · What makes it worse or better
  • Questions I need answered before I leave
  • What I am hoping for (referral, test, treatment, a name for it)
  • Things that help this appointment go well

How to use it

  • 1Print it, or open it on a tablet and fill it with a stylus.
  • 2Use it for one week before judging it. New habits need a few repetitions.
  • 3Bring it to your doctor or therapist: a filled sheet says more than memory.
