Accommodations Request Sheet
What to ask your school or employer for, in their language. Tick what applies, add your own, hand it over.
Most accommodations are cheap and legally supported in the EU, UK, US and Canada once a need is documented. Employers and schools respond to concrete, bounded requests ('extra 25 percent time on written tests') far better than to 'I struggle'. This sheet turns a diagnosis or a screening result into a short, reasonable list.
What is inside
- ✓Reading, writing and information (dyslexia, dyscalculia, processing)
- ✓Attention and executive function (ADHD)
- ✓Sensory and social (autism)
- ✓My specific requests (3 maximum, concrete, bounded)
- ✓Supporting documents I have (diagnosis, screening report, letters)
- ✓Proposed review date and who to contact
How to use it
- 1Print it, or open it on a tablet and fill it with a stylus.
- 2Use it for one week before judging it. New habits need a few repetitions.
- 3Bring it to your doctor or therapist: a filled sheet says more than memory.
This tool supports self-management. It is not a diagnosis and does not replace professional care.
