Autism Assessment Prep Sheet

Social communication, routines, sensory life and masking, described with concrete examples from childhood to now. Write it before the appointment, hand it over.

Adult autism assessments look at social communication, need for sameness, intense interests and sensory differences, present since early childhood. Masking hides all of it from the outside: describe the effort and the cost, not just the result.

How to use it

  • 1Print it, or open it on a tablet and fill it with a stylus.
  • 2Use it for one week before judging it. New habits need a few repetitions.
  • 3Bring it to your doctor or therapist: a filled sheet says more than memory.
