The ACE questionnaire is a 10-question screening tool measuring exposure to traumatic events during childhood (before age 18), including abuse, neglect, and household dysfunction. Developed by Felitti and colleagues at Kaiser Permanente and the CDC, it is one of the most widely used instruments for assessing the long-term impact of childhood adversity on adult health and wellbeing. Answer each question thinking about your first 18 years of life.
⚠️Disclaimer: This is an educational screening tool, not a medical or psychiatric diagnosis. A higher ACE score is statistically associated with increased risk of health and mental health challenges in adulthood, but does not predict individual outcomes. Many people with high scores live healthy, fulfilling lives. If these questions bring up difficult memories or emotions, please consider speaking with a mental health professional.

What is the ACE (Adverse Childhood Experiences) screen?
The ACE questionnaire measures exposure, before age 18, to forms of adversity such as abuse, neglect or a dysfunctional household. Born from the landmark Felitti and CDC study in the 1990s, it showed that an accumulation of difficult childhood experiences often leaves lasting marks into adulthood.
Why it matters
A high ACE score is linked with greater risk of emotional difficulties, chronic pain and health problems in adult life. Understanding your history is not a sentence: it is a starting point for making sense of certain reactions and beginning a process of repair. Many neurodivergent people have also lived through early adversity.
About this test
This questionnaire is free, fast and confidential. It is an awareness tool, not a diagnosis: it does not measure resilience or protective resources. If revisiting these memories stirs up distress, be gentle with yourself and please reach out to a professional or a support line.
